Intake Application Process
Anyone can make a referral to TABIRS. However, we require the following information prior to bringing the request for service to our Intake Committee:
• A completed ABI Community Profile and relevant brain injury medical and consult reports must be included and submitted to the Toronto ABI Network by fax.
The Toronto ABI Network offers a single point referral service to publicly funded member agencies. Our partner organization CHIRS is a member agency of the Toronto ABI Network.

TABIRS services are offered on a subsidized and fee-for-service service basis. All applicants requesting subsidy will be required to complete an income disclosure as part of the application process.
• Those accessing services with third party funding should also complete the above application form, but must forward it with the relevant medical documentation directly to TABIRS by fax, mail or email.
Once all information is submitted, we will contact you to set up a screening interview with an Intake Facilitator who will then bring the request to the TABIRS Intake Committee for review. A letter will be sent to the individual and/or referring agent with the decision of the Intake Committee. If you and/or the referring agent are dissatisfied with the decision of the Intake Committee, we will provide you with information for appeal.

CHIRS has provided me with a greater sense of independence. Before CHIRS, and after my accident, I was living with my mother. Moving into a residence at CHIRS gave me a chance to step back into society. As well, I really love the music program at CHIRS; It gives me a chance to practice the harmonica and to join my friends in singing. I also like to mentor at the club. Doing laundry, serving coffee and washing dishes is a great way to feel productive and give back to CHIRS.
The quality of human beings in this building, from staff to volunteers to students to participants, is so far superior to what is found in the regular world. There is always someone here to lend a hand or provide a positive word when needed. There is always something to do at CHIRS. You will never be bored. Many people who attend have had their life change, going from working full time to having a lot of time on their hands. CHIRS gives us a place to go and meet people and have fun.
CHIRS is a good place to hang out. People are my ‘own type’ in that we all have brain injuries which means that we understand each other. It is good to know that at CHIRS people won’t dismiss me right away if they see that I walk funny or hear me talking in a peculiar fashion. I mentor at the club on Mondays and I do the work as needed. It keeps me busy and helps me feel needed. Plus the coffee is cheap which is a bonus!
My favourite thing about CHIRS is connecting with my peers and helping out. I prepare lunch at the club twice a week, run a Scrabble group on Tuesday and help with karaoke on Thursday. It helps to keep my brain stimulated and allows me to feel like I am doing something worthwhile with my life. I love the challenge of making the club a welcoming and positive place. It feels so good to turn frowns into smiles. If I didn’t have CHIRS I would feel isolated and would have far less to look forward to.

How much does a neuropsychological evaluation cost?
Assessments are billed at an hourly rate consistent with the Ontario Psychological Association’s recommendations. Test batteries and reports are tailored to the purpose of the examination.
Will I be provided a cost before a neuropsychological evaluation?
Written estimates of costs are provided before the examination. Signed approval for billing is required before the date of testing.
What age do I have to be for a neuropsychological evaluation?
TABIRS neuropsychology services specialize in psychometric assessment of adolescents (aged 18 and over), adults and seniors with acquired brain injury for rehabilitation and medical-legal purposes.
How can I make a donation to the CHIRS Foundation?
There are two ways to make a donation. Either donate via the CanadaHelps.org website or alternatively donate by cheque. If using the latter donation method, please print and download the donation form from the CHIRS website. Please follow the instructions on screen.
